Documentation is a central part of health and social care. It is both a right for the service user and a duty for staff.

Why Documentation Matters
Documentation makes it possible to follow up on support, ensure that service users receive the help they need, and improve the quality of care. It also records how support has been provided and whether the service user’s situation or well-being has changed.
Social Documentation
Documentation carried out under the Social Services Act (SoL) and the Act Concerning Support and Service for Persons with Certain Functional Impairments (LSS) is called social documentation. It describes:
- What type of support is provided.
- How the support is delivered.
- Any changes in the service user’s needs, situation, or health.
Service User Rights
Service users have the right to access their documentation. This creates transparency and allows them to follow how their support has been delivered.
Support for Staff
For staff, documentation is a tool that clarifies what should be done, how it should be carried out, and what has already been completed. It ensures continuity and builds trust in the care process.
Rules and Responsibilities
There is a legal obligation to document all health and social care interventions. Documentation must be factual, accurate, and carried out continuously.

- New employees receive a login when they start.
- Training and an introduction to the system are provided.
- Combine ensures that all documentation is managed consistently and securely.
