Implementation plan

The implementation plan describes how support should be delivered and forms the basis of social documentation.

What an Implementation Plan Is

An implementation plan is an agreement between staff and the service user on how support should be provided. It is written together with the service user, often by their designated contact person, and includes:

  • A description of how different activities should be carried out.
  • Any goals linked to the support measures.
  • The service user’s wishes and everyday needs.

Part of Social Documentation

The implementation plan is the foundation of social documentation under the Social Services Act (SoL) or the Act Concerning Support and Service for Persons with Certain Functional Impairments (LSS). It makes it possible to follow up and develop support over time.

How to Use an Implementation Plan

You should read the implementation plan before meeting the service user for the first time. The plan gives you knowledge of what to do and how the support should be provided. Re-read the plan regularly to keep it fresh in your work.

Follow Goals and Agreements

The plan often contains goals that the service user and their contact person have agreed on. It is important to be aware of these goals in order to provide meaningful and tailored support.

Updating the Plan

An implementation plan must be reviewed and updated regularly:

  • At least every six months.
  • Whenever there are significant changes in the service user’s needs or support.

Example of the Importance of an Implementation Plan

Mia has mobility difficulties and becomes anxious when staff do not explain what they are doing during transfers. Her legal guardian requested that it be written into the implementation plan that staff should always explain each step. With this clearly described in the plan, both regular staff and substitutes can work in the same way and help Mia feel safe during transfers.
