What an Incident Is
An incident is an unexpected event that has led to, or could have led to, risk or harm for someone receiving health care, social care, advice, or support.
Purpose of Reporting
- To identify and correct shortcomings.
- To prevent the same issue from happening again.
- To improve and raise the quality of services.
Incident reporting is not about blaming individuals but about learning from mistakes.
How to Report an Incident
In Boden Municipality, incidents are reported in the DF Respons system.
- It can be accessed through the documentation system Combine or via a link on the municipal intranet Insidan.
- Training on how to use the system is also available on the intranet.
Who Should Report
All employees are required to report incidents – both permanent staff and substitutes. The person who discovers the incident is responsible for writing the report.
Examples of Incidents
Common Situations
- A fall.
- Treatment not carried out.
- Missed supervision visit.
- Medication not given as prescribed.
- Medication mix-up.
